New Patients
- We welcome new patients to our practice and hope to serve you to the best of our ability, however, if we are not a right fit for you, we may not be able to do so effectively and efficiently. Please review all our policies to ensure we are a right fit for you and your family.
- Are you pregnant and expecting a new addition to the family? We accept maternal visits after postpartum care with the OB provider and new baby visits.
- We believe in the positive impact of vaccines as a practice and, as a minimal guideline, we expect that all our pediatric patients will be up to date with all American Academy of Pediatrics (AAP) recommended vaccines by age 2. If you choose not to vaccinate your child(ren) according to our guidelines, we respect your decision and will work with you to provide ongoing education, evidence-based information, and supportive care while continuing to prioritize your child's safety and the health of our community.
- If this is your first appointment with us, please arrive 15 minutes early to complete the New Patient paperwork.
- Please check to make sure your insurance or our fee schedule works for you. Be sure to bring your insurance card with you to the visit (if applicable).
- If you or your child are coming from another provider, we ask that you have the medical records transferred to our office in advance of your first visit.
Policies
Missed / Cancelled Appointments
- A fee may be charged for missed appointments.
- Missed appointments represent the time taken away from other patients who could have been seen in the time set aside for you, a cost to us, and a cost to you, so please call to cancel if you will not be able to make your appointment.
- The third missed appointment within a one-year period by a patient or family will result in discharge from the practice.
- Canceled appointments should be called in at least 24 hours before your scheduled appointment to avoid a charge.
Late Policy
- We recognize there may be conditions beyond your control that may cause you to arrive late to your visit. However, out of courtesy to other patients, if you are later than 15 minutes for your appointment, you will be asked to reschedule if we cannot work you back into the schedule.
Vaccine Policy
- As a practice, we believe in the protections vaccines have given the entire population. There are diseases and illnesses that we have forgotten existed or have been drastically reduced because of vaccines. As a minimal guideline, we expect that all our pediatric patients will be up to date with all AAP recommended vaccines (except for flu and covid vaccines) by age 2. If you choose not to vaccinate your child(ren) according to our guidelines, we respect your decision and will continue to manage your child's care; however, a vaccine consultation will be incorporated into routine visits, and a vaccine refusal form will be required to be signed and kept on file.
Telehealth Policy Statement
- Our practice offers telehealth services to improve access to timely, high-quality care when clinically appropriate. Telehealth visits may be used for select acute concerns, chronic disease management, behavioral health, medication follow-ups, and review of labs or imaging, based on provider judgment and patient safety.
- Services are conducted through a secure, HIPAA-compliant platform, and informed consent is required prior to participation. Patients acknowledge that they are aware of the limitations of virtual health and that an in-office evaluation may be recommended when necessary to ensure safe and appropriate care.
- Telehealth is not appropriate for medical emergencies. Patients experiencing severe symptoms such as chest pain, difficulty breathing, altered mental status, or uncontrolled bleeding should call 911 or seek immediate in-person care.
Forms
We are happy to complete forms and write medical letters as necessary. Please allow 5–7 business days to complete each form. Parents should complete all the parents' portion of the form prior to dropping off. Completed forms can be sent back by mail, fax, or picked up in person during office hours or accessed through your patient portal. Due to HIPAA compliance, we cannot send completed forms via email.

Appointments
Emergencies
- If you have an emergency, call 911. Emergencies are those conditions that are life-threatening. When our office is closed and you have an urgent problem that cannot wait for office hours, you can call our main number and the provider on-call will give you advice.
Sick Visits
- Sick visits are scheduled as same day or next day appointments based on availability.
Well Visits
- We recommend that you call our office at least 4 weeks in advance, so you can choose the date and time that best fits your needs.
Well + Sick / Other Visits
- Although not advised, sometimes this may happen. It is best to make separate appointments for other conditions outside of a well visit to ensure enough time is allocated for your issues to be adequately addressed. If unavoidable, we understand, but please note, you may be charged a fee for this visit.
Sports Physical
- Well visits performed before April 1st of the year will need a separate sports physical examination. There will be a fee for this sports physical visit.
All Other Visits
- All other visits will be scheduled based on availability, with your convenience in mind.
