New Patient
- We welcome new patients to our practice and hope to serve you to the best of our ability, however, if we are not a right fit for you, we may not be able to do so effectively and efficiently. Please review all our policies to ensure we are a right fit for you and your family
- Are you pregnant and expecting a new addition to the family? We accept maternal visits after postpartum care with the OB provider and new baby visits
- We believe in the positive impact of vaccines as a practice and as a minimal guideline, we expect that all our pediatric patients will be up to date with all CDC recommended vaccines by age 2. If you choose not to vaccinate your child(ren) according to our guidelines, we respect your decision but ask that you seek care with another practice/provider.
- If this is your first appointment w-ith us, please arrive 15 minutes early to complete the New Patient paperwork.
- Please check to make sure we take your insurance or our fee schedule works for you. Be sure to bring your insurance card with you to the visit (if applicable)
- If you or your child are coming from another provider, we ask that you have the medical records transferred to our office in advance of your first visit.
